Submission Guidelines

  • Cover letter stating the general description of operations, current and desired program, target premium

  • Completed and signed Contractors Supplemental application or equivalent

  • Minimum of 5 years of currently valued company loss runs

  • Minimum of 5 years of historical exposure data (revenue, payroll, etc.)
